*If you are on an HMO plan, be sure to check your benefits information to see if your PCP is required to send a prior-authorization to your insurance company for psychiatric care.
For individuals: If you send a message, our staff will reach out to you gather all the information we need to get you in to see one of our providers as soon as possible.
For facilities: If you send a message, please provide the type of facility you represent (AFH, SNF, hospital, etc.) and your facility name, as well as requested services, in the body of the message. Our staff will reach out to begin making arrangements.
